Title: Occurrence and pathomechanism of mirror movements in acquired central nervous system disorders

Abstract:

Introduction: Mirror movements (RL) are unintended and unaware movements of the limb, which remains at rest, occurring synchronously during performing intentional movements of the contralateral one. Occurrence of marked RL in adults always indicates CNS pathology. Aim of the study: Evaluation the incidence and severity of acquired, clinically assessed RL and the intensity of bioelectrical activity of mirror muscle in patients with unilateral (ischemic stroke) and bilateral (multiple sclerosis, amyotrophic lateral sclerosis) CNS pathology compared to control group, with determination of pathomechanism of RL in patients with acquired CNS damage. Material and methods: The study included 78 patients and 31 healthy people (control group). Patients group consisted of patients with the diagnosis of SM (n = 25), SLA (n = 36) and ischemic stroke (n = 17). The clinical and electromyographic assessment of acquired RL and bilateral transcranial magnetic stimulation evaluating excitatory and inhibitory functions of brain cortex were performed in all subjects. Results: Clinically assessed RL occured with higher frequency and had more greater severity in patients. Bioelectrical activity of mirror muscles was recorded in all subjects, but the mean value of motor unit potentials amplitude was higher in group of patients.Conclusions: The new pathogenetic hyphothesis was created: simultaneous sho ; rtening of duration of interhemispheric inhibition in ipsilateral to RL hemisphere and intrahemispheric inhibition in contralateral to RL hemisphere are responsible for RL generation.
