The aim of the study was to show that patients with isolated relaxation disorders (without systolic dysfunction), which are considered to be the mildest degree of diastolic dysfunction, may develop heart failure with preserved ejection fraction (HFPEF). In a study group of 210 patients with isolated relaxation disorders, 42% of the patients had HFPEF. We showed associations between isolated relaxation disorders and echocardiographic markers of increased left ventricular end-diastolic pressure, elevated brain natriuretic peptide (BNP) levels, and clinical symptoms of heart failure. Moreover, we observed a positive correlation between an insulin dose and BNP levels in a subgroup of patients with type 2 diabetes. Hypothetically, high levels of endogenous insulin in obese patients with type 2 diabetes, who additionally receive insulin, may lead to the development of HFPEF symptoms, which requires further study.
