Tytuł: The use of the plasma leucocyte elastase, calprotectin, lactoferin and intestinal fatty acid binding protein concentration in diagnosis and assessment the clinical activity of Crohn disease and ulcerative colitis

Abstrakt:

Inflammatory bowel diseases are chronic diseases of gastrointestinal tract, which include Crohn’s disease and ulcerative colitis. About 15% of cases are difficult to diagnose using clinical, radiological and histopatological methods. In this study three neutrophillic proteins (leukocyte elastase, lactoferrin, calprotectin), intestinal fatty acid binding protein and neopterin were evaluated as biomarkers used in the diagnosis and the assessment of clinical activity of Crohn’s disease and ulcerative colitis. Additionally a set of antibodies was evaluated. The study included 27 patients with Crohn's disease, 33 patients with ulcerative colitis and 20 healthy controls. Plasma concentrations of the studied proteins and the presence of antibodies were compared between the studied groups.. Plasma calprotectin, lactoferrin, elastase, I-FABP and neopterin concentrations were measured using ELISA. ANCA, ASCA, LFS, GAB, CUZD1 and GP2 antibodies were detected using indirect immunofluorescence. In patients with both inflammatory bowel diseases higher plasma concentrations of leukocyte elastase, calprotectin and lactoferin than in control group were found. Diagnostic characteristics of these tests indicate their usefulness in the IBD diagnosis. Significantly higher elastase/NEU and lactoferrin/NEU ratios in patients with ulcerative colitis as compared to patients with Crohn's disease can be ; useful in the differential diagnosis of these diseases. There was no significant increase neither in I-FABP, nor in neopterin levels in patients with IBD. Plasma calprotectin and lactoferrin levels were higher in patients with the active phase of Crohn's disease as compared to patients in remission, indicating the usefulness of these tests in the assessment of disease activity. The panels of antibodies ASCA & ANCA, and ASCA, ANCA, GAB, LFS, CUZD1 & GP2 may be useful for the differentiation of Crohn's disease and ulcerative colitis.
