Tytuł: Application of cerebral oxygen saturation measurement to optimise general anaesthesia during major surgical interventions within abdominal cavity

Abstrakt:

Introduction Numerous publications indicate that near infrared spectroscopy is clinically applied to determine the local oxygenation.Objectives Evaluation of the incidence of cerebral desaturation during anaesthesia in the patients during major surgical procedures pursued within abdominal cavity. Materials and methods The study covered 101 consecutive, non-randomised patients aged 35 - 84 years (mean age: 63.64 ± 10.42), hospitalised in the Department of General Surgery, Oncology and Gastroenterology, Jagiellonian University School of Medicine, referred to major surgical intervention within abdominal cavity.Results In 14 (13.9%) out of 101 patients who underwent surgical intervention a clinically significant reduction in cerebral oxygen saturation was noted.In 50 (49.5%) of them, apart from general anaesthesia,epidural anaesthesia was also used. Nine of them experienced a clinically significant reduction in cerebral oxygen saturation. There was no statistically significant correlation between the application of epidural anaesthesia and the incidence of cerebral desaturation.Conclusions 1. During major surgical procedures pursued within abdominal cavity the reduction of cerebral oxygen saturation occurred relatively frequently (in 13.9 % of patients), whereas an improvement of cerebral oxygen saturation could well be achieved through pharmacotherapy.2. The application of epidura ; l anaesthesia did not result in a clinically significant reduction in cerebral oxygen saturation during general anaesthesia administered at any major surgical procedures pursued within abdominal cavity.
