Abstrakt:

It is a questionnaire-based cross-sectional study, part of an international project assessing quality of Primary Health Care in Europe (QUALICOPC). In Poland in 2012, random sample of 2200 adult PHC users, visiting 220 PHC physicians from three voivodships were recruited. 1980 of them participated in the arm “Experiences” and 220 in the arm “Values”. A set of indexes was developed to measure the quality of PHC: Satisfaction Index, Value Index and Met Expectations Index. The participants positively assessed quality of PHC. They were most satisfied with the quality of services, accessibility and equity of care. Satisfaction was the lowest for comprehensiveness of care. Older patients, with lower incomes, less educated, with chronic diseases were more satisfied. Similarly, higher satisfaction was observed in patients visiting doctors in rural areas and those more experienced in PHC, and those who had (amongst others) specialty in family medicine. The highest expectations considered accessibility and equity of care, the lowest – continuity. Patients’ needs were fulfilled in all major quality areas. Single unmet expectations were involvement of the patient in decision making and possibility to discuss non-medical problems with physician. Those topics should be a priority in quality improvement. Interpersonal continuity of care seems to be a key element in good quality PHC services.
