Title: Dislocation after total hip arthroplasty

Abstract:

A significant complication burdened with therapeuticproblems is recurrent dislocation after Total hip arthroplasty(THA). The incidence of THA dislocations ranges from 0.5to 20% and is multifactorial.Aims:To define the causes of THA dislocationTo propose an algorithm of care in dislocationTo evaluate treatment options and outcomesMaterial and methods: During 2001-2010 44 patients with68 dislocation episodes of THA dislocations were treated.All patients after their initial dislocation were treated nonoperatively.A 2 dislocation episode was an indication forsurgical intervention. Patients were scheduled for revisionsurgery after at least two episodes of dislocation, or if anysignificant anatomical or morphological causes ofdislocation were identified.The results of treatment wereevaluated using HHS, WOMAC and X-ray images.Results: The incidence of dislocation was 1.68%.Significant factors were: male sex, advanced age, indicationfor arthroplasty. The analysis of component positioningrevealed correct position in 60% of patients. The subgrouptreated by closed reduction had good HHS and satisfactoryWOMAC scores; the revision arthroplasty group hadsatisfactory scores in both scales. After conservative andsurgical treatment, 88.63% patients had no furtherdislocation episodes and the implant was stable.Conclusions:Causes: malposition and loosening; advanced age, male sex,and previous hip surgery.In dislocation of THA with correct implant position, closedreduction resulted in long-term outcome in 50%.The use of the anti-dislocation ring is an effective andminimally invasive option.
