Tytuł: Usefulness of CT in diagnostics and monitoring of GIST treatment
Abstrakt:
The purpose of this study is to evaluate the role of CT in diagnostics and treatment of GIST, including:• determination of usefulness of CT in detection of primary tumor,• determination of the pattern of metastatic spread of GIST in CT,• evaluation of changes in tumor tissue during therapy with Imatinib with particular emphasis on the progression pattern,• development of radiological algorithm for CT evaluation in GIST treated with molecular targeted therapy.Materials and methodsClinical data of 182 patients with GIST and CT images of 59 primary tumors, 60 cases of metastatic spread, including 56 cases before and during treatment with Imatinib and 36 of disease progression were analyzed. The tests were performed in a multi-phase technique, using a 16-row Siemens Sensation 16 CT scanner with the standard preparation of the patient.Results and conclusions The role of CT in the detection of primary tumor was inconsiderable. Only 7% of tumors were detected during CT. Difficulties in interpreting CT images appeared in about half of primary tumors investigated during further examinations (43%). GIST spread mainly to liver and peritoneum, what limits the need of CT examinations of areas other than abdomen and pelvis. Application of Choi criteria instead of RECIST improved evaluation of response to treatment and led to verification of false progression picture (20% of all cases). Use o ; f own modifications simplifies and shortens the procedure of response evaluation in CT.A new type of progression, the so-called "nodule within the nodule" occurred in 53% of cases.
