Tytuł: Natural history and results of surgical treatment of spine deformities inpatients with spinal muscular atrophy
Abstrakt:
Spinal muscular atrophy is a group of diseases manifesting in muscleweakness. The orthopaedic problem is spine deformity present in allnon-ambulant patients. Aim of paper is to determine principles oftreatment of spine deformities in this group. Material consists of 105patients with SMA treated with Galveston-Luque technique. Thenatural history and risk factors of curve progression were evaluated.Results of surgery were analysed and indications for surgery defined.Follow-up was 3.6 years. Age of deformity onset was 6 years. Pace ofprogression was 12°/year; non-operative treatment was ineffective.Risk factors of curve progression were: curve magnitude, pelvicobliquity and L4 angle. Surgery was performed at the age of 11.4years. Curve magnitude preoperatively was 106°. Correction aftersurgery was 49%, at follow-up 41.1%. Mean loss of correction was7.9%. Complications occurred in 30,5% of patients. Good result oftreatment was obtained in 66.7% of patients. Good outcome wascorrelated with following parameters: flexibility on “traction” films,thoracic kyphosis, coronal balance, number of fused vertebrae andpulmonary function. Spine deformities in all non-ambulant children isprogressive, surgery is the recommended treatment, regardless the age.The Galvseton-Luque technique is an effective procedure in thisgroup. The result of surgery is permanent, and the number of seriouscomplicatio ; ns low.
