Title: Optimalization of anti-ischemic therapy in patients with non ST- segment elevation myocardial infraction

Abstract:

Non ST-segment elevation acute coronary syndromes (NSTE ACS) are the major cause of hospitalization of patients with coronary artery disease (CAD). Despite improvement of treatment the short-term prognosis and long-term outcome of those patients remain poor. The administration of glycoprotein GP IIb/IIIa blockers has shown beneficial in recent clinical trials both in conservatively treated patients and after percutaneous coronary interventions (PCI). Another option for use of GP IIb/IIIa blockers with unfractionated heparin are direct thrombin inhibitiors with widely used bivalirudin. The aim of this trial was to compare the efficacy and safety of different anti-thrombotic regimens in treatment of troponin-positive NSTE ACS. One hundred patients enrolled in the trial were randomized to two treatment arms: integrillin (GP IIb/IIIa blocker) with unfractionated heparin or bivalirudin. We found statistically significant difference in the size of the infarct evaluated with enzymatic curves in favour of GP IIb/IIIa inhibitor + unfractionated heparin group. There were no differences between two groups in coronary flow after PCI in TIMI scale in infarct-related artery and the incidence of bleeding complications. The combined end-point of death, stroke, repeated myocardial infarction and bleeding complications occurred with no significant difference between the group during in-hospital, ; one month and six months follow-up period.
