Tytuł: The effectiveness of atrial vs. atrio – ventricular stimulation in sick sinus syndrome in a long term observation

Abstrakt:

Patients with sick sinus syndrome can be treated with AAI or DDD pacemaker implantation. The aim of the study is to analyze the efficacy and safety of both types of stimulation among patients form Cracow’s Specialist Hospital Pope John Paul II in the survival, development of atrial fibrillation and re-operation.In the period from January 2003 to June 2010, 1,355 patients with SSS and intact AV conduction who underwent primary pacemaker implantation were retrospectively analyzed. AAI (R) were 86 in the group DDD (R) 723 patients. Median follow-up was 52mc ± 25 (AAI-65mc ± 27; DDD-50mc ± 24). In AAI group died 13 (15.1%) patients vs 92 (12.7%) patients in the DDD group, p = 0.45; Urgent reoperation underwent in the AAI group 23 (27%) vs 62 (8.5%) patients DDD group p> 0.01. The operative mode change done in 13 patients (15%) in the AAI vs 1 (0.14%) in the DDD p> 0.01. AAI electrode complications discover in 6 (6.9%) vs 49 (6.7%) DDD. The development of atrial fibrillation, 8 (9.3%) in the AAI vs 81 (11.2%), p = 0.23 DDD, the risk factors in development of atrial fibrillation were tachy brady syndrome - 11.31 HR and not receiving antiarrhythmic drugs HR 4, 23, the development of atrial fibrillation and mortality did not differ between the two groups. The risk of reoperation was slightly higher for patients in the AAI.In patients with sick sinus syndrome there is no statistically s ; ignificant difference between the deaths and the presence of atrial fibrillation. Increased risk of reoperation due to operational necessity of ventricular upgreade due to development of AVB and the AF with bradycardia prove that AAI pacing is not as safe as DDD, so that the atrio – ventricular pacing should be more common in patients with SSS.
