Title: The influence of hyperglycemia on myocardial reperfusion in myocardial infarction treated invasively

Abstract:

The aim of this trial was to investigate a relationship of admission hyperglycemia ( 7,8 mmol/l) and elevated next-day fasting glucose to myocardial infarct size and to reperfusion in STEMI patients treated with PPCI.In the admission glucose part of the study 76 patients were enrolled. Patients with hyperglycemia ( 7,8 mmol/l) had larger infarct size than patients without hyperglycemia. The trend towards worse ST-segment elevation resolution was observed in hyperglycemia group.In the second, next-day fasting glucose part of the study 80 patients were enrolled. They were divided into two groups based on median fasting glucose level (6,6 mmol/l). Patients with higher fasting glucose levels had significantly larger infarct size and worse myocardial reperfusion, as assessed by amount of complete ST-segment elevation resolution 60 minutes after PPCI.Additionally, patients with admission hyperglycemia and patients with higher fasting glucose were more frequently diagnosed with new diabetes and with abnormal glucose regulation based on oral glucose tolerance test done before hospital discharge. Both worse achieved reperfusion and larger myocardial infarct size, appear to be important factors connecting states of hyperglycemia with adverse outcomes in STEMI. Future trials should ultimately answer the question, whether rapid reduction of hyperglycemia in STEMI has a potential to impro ; ve patients’ outcomes.
