Abstract:

The aim of the study was to asses the types of saccadic eye movement abnormalities, the effect of both levodopa and STN DBS treatment on saccadic eye movement abnormalities in the advanced PD patients, as well as the correlation between parkinsonism symptoms and saccadic eye movement abnormalities under the applied treatment (STN DBS, levodopa) in the advanced PD patients. The study included 44 patients with idiopathic PD in the advanced stage treated with levodopa, 20 patients with a diagnosis of idiopathic PD after the deep brain stimulation procedure, and 48 healthy controls. The saccadic eye movement were recorded with a saccadometer. Patients were examined twice: in the clinical "off" and "on" condition. The control group was tested once.The results of the study confirm the previous reports on saccadic eye movement abnormalities in the advanced Parkinson’s disease in the ‘off’ condition. With the neurostimulator on, reflexive saccades in DBS patients are improved. However, voluntary saccades show little improvement. With the neurostimulator on, the saccadic parameters changes confirm automatic responses inhibition dysfunctions in PD patients. Levodopa treatment in patients treated only pharmacologically results in the opposite effect on saccades in comparison with neurostimulation turned on. Saccades get worsened with respect to the ‘off’ condition. Automatic response inhi ; bition is reinforced. The only correlation between aggravated parkinsonism and saccadic eye movement parameters indicates how neurostimulation enhances bradykinesia and aggravates automatic response inhibition.
