Title: Disordered calcium - phosphate homeostasis and nutritional status in children with newly diagnosed inflammatory bowel disease


Introduction: Inflammatory bowel disease (IBD) is one of the most common chronic childhood diseases. Objectives.: The aim of this study was to investigate disturbances of nutritional state, calcium – phosphate homeostasis of children with IBD.Materials and Methods: The study was designed as a prospective project. Patients, diagnosed with IBD, at the Department of Pediatrics, Gastroenterology and Nutrition of Polish – American University Children`s Hospital in Cracow, between 2009 and 2011 year were submitted to the study. Assessment of growth failure and weight loss was measured, as well as, calcium – phosphate homeostasis, and bone mineral density.Results: Fifty patients were included into the study. Crohn`s disease was diagnosed in 35 children, ulcerative colitis in 15 patients. Cole index (CI) was below 85% in 42,55% children with IBD. After a year CI was higher in all patients.Reduced 25(OH)D levels, were present in 87,23% patients. After one year, vitamin D level was not significantly decreased and was observed in about 93,6 % of children with IBD.Z-score BMD, below -2 SD, was recognized in 25,53% children, more frequently in patients suffered from Crohn`s disease. After one year, Z-score BMD was higher in the whole population, but only in the female changes were statistically significant. Conclusion: Disturbances in bone mineralization are a frequent complication of IBD. ; There is a need of diagnostic protocol which would allow a prevention, early diagnosis, monitoring and treatment metabolic bone disease in children with IBD.
