Title: Morphometric evaluation of the hormonal expression of the pituitary adenomas operated via transsphenoidal route in relation to the clinical picture
Abstract:
There are some discrepancies in the literature between immunohistochemical staining results and clinical picture of pituitary adenomas. The aim of this study is to evaluate the relationship between the pituitary adenomas hormones immunoexpression in comparison with endocrinological picture, the plurihormonality of the pituitary adenomas as well as the reliability of the histopathological material. The cases of 72 patients, who underwent transsphenoidal surgery for pituitary adenomas between 2004 and 2009 were analyzed. As a result, 22 monohormonal adenomas (30,56%), 21 plurihormonal adenomas (29,17%), 21 immunonegative adenomas (29,17%) and 8 unreliable cases (11,11%) were recognized. Statistically significant relation between immunohistochemical picture and preoperative diagnosis was found, respectively, for: PRL andhiperprolactinemia, GH and acromegaly, ACTH and Cushing syndrome. Statistically significant impact on reliability of the histopathological material had presence of hemorrhages, neurohypophysis, necrosis and its quantity. The lack of absolute concordance between clinical and immunohistochemical picture results mostly from presence of the nonfunctioning and plurihormonal adenomas and of unreliable specimens. An immunohistochemical staining for all adenohypophyseal hormons is obligatory for correct classification of pituitary adenomas. The awareness of an operating ne ; urosurgeon of the importance of collecting histopathological material has essential impact on further neuropathological evaluation and possibility of immunohistochemical staining.
