Tytuł: Change in selected risk factors of atherosclerosis in the group of adolescents on the threshold of adulthood

Abstrakt:

Introduction: The favorable changes in the CV morbidity and the mortality, that have taken place in Poland are insufficient to reduce epidemic level comparable to other EU leading countries. Aim of the work: Evaluation of changes of selected risk factors of atherosclerosis in the group of adolescents on the adulthood threshold.Materials and methods: The study group consisted of 100 women and 41 men, students of the two last secondary schools classes. The group was examined twice, with one year gap, using the same methods (the diagnostic survey, the anthropometric measurements, the blood tests). 16 potential risk factors for atherosclerosis were selected.Results: Average age of the respondents was 17.85 0.51 years and 18.96 0.51 years in the second one. The occurrence of CVD among the relatives was reported by 46.8% of the group. The average number of the CVD risk factors was high, similar in women (4.88 1.88) and men (5.07 1.23); no significant difference between two examinations and the genders. The frequency of hypertension increased (from 4.3% to 11.3%, p = 0.000). MS was diagnosed in 47.5% of the adolescents in the first examination and 49.7% in the second one. The considerable rise was noted in the concentration of the hsCRP (p = 0.044), fibrinogen (p=0.000), TC (p=0.005), LDL-C fraction (p = 0.008) and also in HDL-C (p=0.000), the values of BMI, WHR and waist size (p<0.05 ; ). The body fat composition measured with the Tanita weight increased in the men group (p=0.005), while in the women group the considerable rise was noticed with the Omron measurements (p=0.023). An increased tension due to situation at home was reported by the 14.9% of respondents and 17% in the second examination, increased tension at school respectively: 28.4% and 24.8%. 5 or more depression symptoms, lasted for at least 2 weeks during the previous 12 months, were identified by 8.5% of the students and by 9.9% in the second examination. The number of declared active smokers decreased (p=0.012). Around 80% of respondents drank alcoholic beverages and ate irregularly. The knowledge about the atherosclerosis risk factors and ability to develop CV self-control increased significantly (respective p=0.022 and p=0.000).Conclusions:1. Over 90% of the students showed at least 3 out of the 16 analyzed risk factors, nearly half had the CVD positive family history.2. The situation worsened, due to a significant increase of CVD related risk factors as the frequency of hypertension, anthropometric measures (BMI, abdominal obesity: WHR and waist size, fat body composition) and biochemical measures (hsCRP, fibrinogen, TC and LDL). Rise of HDL-C was a single positive change.3. The significant reduction of selected CV risk factors was observed.4. In relation to the selected risk factors, the ; changes in the group of men and women were not unidirectional.
