Tytuł: Transcatheter closure of atrial septal defect with the Amplatzer Septal Occluder

Abstrakt:

The study aimed to evaluate the safety and effectiveness of the transcatheter closure of ASD Material and methods 35 subjects (23F, 12M; mean age: 42.7 ± 12.7) were analysed. The control group comprised 19 healthy volunteers. In all pts before and after procedure: clinical study, transthoracic echocardiography, 24h, cardiopulmonary exercise tests (CET), determination of Endothelin 1 (ET1) level ECG monitoring were repeated. Results: The ASO devices were successfully implanted in all subjects. There was improvement of NYHA and CET parameters after 6 and 12 m-ths (pts up to 40 y benefited more). The increase in the number of supraventricular ectopy (SVE) was noted 1 month after the procedure with a regression in within the follow-up. 1 month after the procedure decrease of right heart dimension were observed. ET1 levels in subjects with ASD were higher than in the healthy pts. There was a significant decrease of ET1 level within 48 hours after ASD closure. Conclusions: 1. Transcatheter closure of ASD is a safe and effective procedure and caused significant improvement of haemodynamic parameters and overall clinical status. It resulted in a improvement of exercise capacity as early as 6 months after the procedure and younger pts benefited more. 2. The procedure is associated with a transient increase in supraventricular premature beats in the early follow-up. There is a re ; gression of periprocedural arrhythmias after 6 months of ASD closure. 3. Plasma Endothelin 1 level in the peripheral artery and in the pulmonary artery may be considered as one of the predictive parameters in diagnosing and subsequent referrals for the transcatheter closure of ASD.
