Title: Fibrin clot structure and its susceptibility to fibrinolysis in patients with Buerger disease and peripheral atherosclerotic occlusive disease

Abstract:

Abnormal fibrin clot structure and hypofibrynolysis may be important in several diseases including advanced coronary artery disease. It is not known if decreased permeability and resistance for lysis of the clots can also be found in patients with peripheral artery occlusive disease (PAOD) and with inflammatory artery diseases like Buerger disease either. The aim of the study was to evaluate fibrin clot properties, such as permeability and susceptibility to fibrinolysis, and identify factors which influence these variables in patients with peripheral atherosclerotic occlusive disease and Buerger disease. 55 consecutive patients with PAOD aged 44 to 70 and 16 patients with Buerger disease aged 38 to 56 years were enrolled to the study. Control groups consisted of 30 and 17 healthy age-matched volunteers, respectively. In both patient groups (PAOD and Buerger) the grade of the lower extremity ischaemia was assessed. In all patients, blood samples were drawn for the measurement of lipids, concentration of fibrinogen, C-reactive protein, interleukin-6, intercellular adhesion molecule-1 (ICAM-1), plasma and cellular fibronectin, fibrinopeptide A, tissue plasminogen activator antigen and plasminogen activator inhibitor-1 antigen. Fibrin clots obtained from citrate plasma samples were used for the measurement of clot permeation, expressed by the permeability coefficient, Ks (Darcy con ; stant). The susceptibility to fibrinolysis of the clots was assessed by serial measurements of D-dimers in the buffer percolating through fibrin gels after addition of recombinant tPA. The factor XI11 Val34Leu polymorphism was also determined. This study demonstrates that decreased permeability and hypofibrinolysis of fibrin clots, previously described in patients with coronary artery disease and their relatives, also characterize patients with symptomatic, advanced peripheral artery occlusive disease. For the first time, it has also been shown that exceptionally low permeability of the fibrin clots and hypo fibrinolysis (compared to healthy controls and patients with PAOD) occur in patients with Buerger disease. Markedly increased levels of cellular fibronectin in patients with Buerger disease could be useful in the differentiation of this disease from peripheral atherosclerotic occlusive disease.
