Tytuł: Composite restorations with the inserts : clinical and laboratory investigation

Abstrakt:

A problem which still remains unsolved is the polymerization shrinkage of composite materials. One of the techniques which reduce the polymerization shrinkage is the use of composite inserts. This method reduces the total quantity of resin to be cured and thus results in less polymerization shrinkage and limited marginal microleakage. The goal of this study was to assess the quality of fillings made of composite materials applied either with the conventional or the inserting method under both clinical and laboratory conditions. 295 composite fillings were made either with the conventional or with the inserting method in class I and II defects in 117 patients. As filling material two composites were used: classical microhybrid Filtek Z250 (3MESPE) and with the addition of prepolymerized filler Gradia (GC). The fillings were assessed using the USPHS scale. The laboratory examination was performed on extracted molars. The fillings were made in class I and II defects with the same protocol as in the clinical study - 8 test groups of 10 samples and 1 control group (healthy teeth). After 24 hours the samples were exposed to 2000 thermocycles. The measurement of the destroying force and of the destroying energy was performed. 8 numerical models were made in the tooth 16 in normal occlusion each corresponding to one of the 8 study groups. Analysis of reduced stress d ; istribution and of main deformations was performed. The percentage of clinically accepted fillings after 24 was 94%. The quality of fillings made with the inserting method was significantly higher compared to the conventional one, irrespectively of materials used or class of defect. Addition of pre-polymerized composite inserts resulted in the reduction of strength parameters ( except for class I fillings made with Filtek Z250) and of reduced stresses in both classes of fillings, in the increase of main deformations in class II fillings and in the decrease of main deformations in class I fillings. The lower values of mechanical parameters of the fillings with inserts did not influence the quality of composite restorations in the two-year-observation period.
