Abstract:

There are no strict guidelines concerning the tension and twisting applied to the patellar tendon graft (B-PT-B) when anterior cruciate ligament (ACL) reconstruction is performed. The aim of the present study was to answer the following questions: Does 180° external twisting of the B-PT-B influence on anterior laxity of the knee, compering to the lack of the rotation? Can 180° external twisting of the graft enable to decrease the applied tension without increasing the laxity? Can the direct results in the cadaver model be transferred into the clinical work? The study was conducted with the cadavers, excising ACL and then replacing it with the B-PT-B. Every graft fixation was repeated 4 times in different rotation and tension combination: 0°-50N, 0°-100N, 180°-50N, 180°-100N. Laxity was measured with KT-1000 tensiometer. The laxity results of the cadavers were also compered with the results of the control group. The results were similar in all three knee measuring positions: full extension, 30° flexion, 90° flexion. 180° external rotation does not influence on the knee laxity, compering tension 50N and 100N. The rotation does not let decrease the applied tension without the increase in the knee laxity. Only 100N tension is enough to restore the previous laxity. The direct results from the cadaver model cannot be transferred into the clinical work.
