Title: Usefulness of noninvasive assessment of pulmonary hemodynamics after administering inhaled nitric oxide using BCTS method in patients with pulmonary hypertension

Abstract:

The aim of the study was assessment of usability of noninvasive pulmonary vasoreactivity test. The test group consisted of 42 patients (26 with arterial, 16 with pulmonary venous hypertension). To define the effective dose of nitric oxide (NO), the first 10 patients were administered NO in the increasing doses of 10, 20, 30 ppm. Based on the obtained results, the effective dose of NO was determined at 20 ppm. After NO inhalation (20 ppm) in patients with pulmonary arterial hypertension a considerable drop with pulmonary arterial pressure by 9% and in pulmonary vascular resistance by 15.8% was observed. The criteria for positive test were obtained with 5 patients (41.7% of patients with idiopathic pulmonary hypertension and 19.2% with pulmonary arterial hypertension). In group with pulmonary venous hypertension a considerable drop in mean pulmonary arterial pressure by 9.1% and in pulmonary vascular resistance by 25% was observed. The criteria for positive test were obtained with 7 (43.75%) patients. No side effects during NO inhalation were observed. An analysis of the correlation between the results of invasive and noninvasive response test pointed to the following relationships: RVSP vs sPAP (r=0,954); RV-IVRT vs sPAP (r = 0,458); ΔRVSP vs ΔsPAP (r=0,457); ΔRV-IVRT vs ΔsPAP (r = 0,557). Increased concentration of endothelin-1 and natriuretic peptides was observed in both grou ; ps. Considerably increased levels of ET-1 and pro-ANP were observed in the group with venous pulmonary hypertension, while in the group of patients with idiopathic arterial pulmonary hypertension there occurred a considerable increase in NT-proBNP.
