Tytuł: Assessment of the effectiveness of a rehabilitation programme in Farmers with low back pain

Abstrakt:

The purpose of this research was to analyze the outcomes of The Health Resort treatment approach in the management of degenerative Low Back Pain (LBP) in Farmers. The Oswestry Disability Index (ODI) was completed and physical measurements (range of movement (ROM) of lumbar spine, strength and endurance of trunkal muscles, symmetry ratio of back extensors) were carried out immediately prior to, and after the Rehabilitation programme of 21 days duration. There had been significant improvement reported on ODI in the female group, but no change reported in the male group, although examination of biomechanical parameters showed considerable improvement. The result of the research demonstrates improvement in functional status, that was expressed in an increase in ROM of the lumbar spine assessed with goniometry, “finger floor” test and Schober`s test. On examination prior to beginning the Rehabilitation programme, some of the patients presented with right and/or left positive Straight Leg Raise (SLR) that was reduced (increased pain-free angle of SLR) or absent when assessed immediately at the end of programme. The maximal and relative muscle torque was used to describe the strength of flexor and extensor muscles of the trunk during the isometric muscle contraction. There was also found to be a greater improvement in the strength of the abdominal muscles relative to the increase in t ; he back extensor muscles. No change was noted in endurance of the back extensors. The muscle activity of the back extensors in Farmers with degenerative LBP is characterized by greater right sided asymmetry of muscle tone at rest, and slightly smaller asymmetry in unsupported sitting. It was observed that activities that generated greater muscle tone (forward flexion, maximal muscle strength test and endurance test) were found to create almost symmetrical back extensor activity. There was found to be no correlation between symptoms of LBP and the patient’s age, years of work in farming, duration of symptoms and type of previous medical and therapeutic intervention.
