Gastric cancer is one of the most frequent malignant neoplasms in Poland and worldwide. The prognosis for gastric cancer remains poor, mostly due to the delayed diagnosis and surgical treatment in majority of the cases. In the dissertation the author investigated factors that may influence the time of delay of gastric cancer diagnosis and treatment. A study group consisted of 959 patients. All patients were interviewed using specially designed questionnaires. The results obtained that 96.3% patients with early gastric cancer, and 92.9% with advanced disease had subjective and objective symptoms. The symptoms persisted through 2-6 months before the first visit to the doctor. Ignoring initial symptoms by patients was one of the main reasons of delayed diagnosis. Gastric cancer does not produce typical subjective symptoms, however, careful interview may reduce the time of doctor’s delay. It usually takes 2.3 months since the first visit to the doctor until gastroscopy examination, which shows that doctors frequently falsely interpret and ignore symptoms reported by the patients. Most effective measure to improve early detection of gastric cancer is performing endoscopic examination in all patients, who report the initial upper gastrointestinal symptoms. An important role in the early diagnosis of gastric cancer is also attributed to the appropriate endoscopic evaluation of s ; tomach mucosal. In the analyzed group of patients, early gastric cancer was diagnosed macroscopically during the first endoscopic examination in only 50.6% of patients, which may point to inadequate qualifications of the endoscopists. Mean time since the diagnosis of gastric cancer until surgery was 1.4 month. Surgery is most delayed in the patients, who are in generally poor condition or suffer from other diseases.
