Tytuł: PCI results in type 2 diabetic patients
Abstrakt:
The study aimed to evaluate the effect of metabolic inflammatory disorders on the results of PCI in diabetic patients with coronary artery disease. 102 consecutive diabetic patients 70 men and 32 women aged 60.7±8.7 years who underwent elective PCI were studied. During one year follow-up clinical, metabolic and inflammatory parameters were analysed in all patients as well as MACE rate was monitored. Results: During follow-up there was a tendency toward improvement of control of studied parameters. The prevalence of early complications was small and MACE occurred in 32 patients (6 died, 5 suffered from non-fatal myocardial infarction, and 22 had restenosis) Conclusions: 1. In about 50% of diabetics metabolic and inflammatory parameters are well controlled and they are only slightly improved in follow-up. 2. Early results of elective PCI in diabetics are good and in follow-up 30% of patients suffer from MACE despite multifactorial treatment and regular visits. 3. Small LDL and HDL level, high triglycerides and AGE level are restenosis risk factors. High baseline hs-CRP level and increased albuminuria elevate the risk of death and myocardial infarction. High insulin level and insulin-resistance affects both the risk of restenosis and death or myocardial infarction. 4. Periprocedural insulin level has the strongest influence on the results of PCI and predictive value in diabe ; tic patients. 5. Complex metabolic control in diabetic patients significantly decreases MACE rate in long-term fellow-up after PCI.
