Tytuł: Cardiotoxicity of paracetamol in acute poisoning
Abstrakt:
Although there were numerous case reports on cardiotoxicity of paracetamol, there were no clinical studies evaluating its cardiotoxicity in acute poisoning. The study group consisted of 33 patients (9 men and 24 women) of Department of Clinical Toxicology CM UJ in Kraków acutely poisoned with paracetamol. The clinical course of poisoning (according Poisoning Severity Score) was mild in 13 patients, moderate in 12 patients, and severe in 8 patients. There were no deaths due to the poisoning. AST and ALT level, total bilirubin and INR were significantly higher in the study group comparing to the controls. cTnI level was less then O, 1 µg/ml in both subjects and controls. Mean blood pressure (24 ABPM) was similar in both groups. The mean heart rate, the maximal heart rate and the minimal 24h heart rate were also similar in both groups. There were no ventricular tachycardia episodes or ventricular rhythms. The other analysed rhythm abnormalities in both groups were not statistically different. The ejection fraction (in echocardiography) measured in acute phase of poisoning was within normal ranges in all patients and rose not significantly at the end of hospital stay. Mild hypokinesis was seen in one patient. G-SPECT using 9 mTc-MIBI was done at rest in 20 patients 3 to 5 days following the poisoning; normal homogenous myocardial label uptake was found only in 2 of them. T ; here were no "cold" focal lesions typical for myocardial infarction. Systolic function abnormalities were seen in 14 of 20 patients. In acute paracetamol poisoning primary cardiotoxic effect can be seen of minor clinical significance.
