Tytuł: Short and long term cardiogenic shock in myocardial infarction treatment results
Abstrakt:
Background: Advance has been made in therapy of myocardial infarction over last years. There is high mortality rate in cardiogenic shock. Evaluation of usefulness of new methods of combined invasive and pharmacological therapy is required. Aim: Aim of this study was to evaluate results of combined interventional therapy in cardiogenic shock (CS) complicating myocardial infarction. Methods: 202 patients with CS treated with early invasive strategy in years 2000-2005 were included into analysis. Demographic, clinical and angiographic data were analyzed to evaluate their influence on therapy results. Results: Study group contained 31,7% women and 68,3% men, mean age was 63,66 years (± 10,91). Direct result of angioplasty was TIMI 2 or 3 and was achieved in 67,97% of patients. Microcirculatory blood flow MPG 2 or 3 was achieved in 36,47% of patients. In-hospital mortality was 56,43%, whereas 1-year mortality was 59,40%. Major adverse cardiac events in 1-year follow up were: rehospitalization 61,90%, reinfarction 6,97%, rePTCA 7,14%, CABG 4,76%. Survivors and non-Survivors were compared to establish factors influencing the outcome. Patients with worse TIMI and MPG had higher mortality rate. In TIMI 3 mortality rate was lower and reached 24,36%. When PCI failed (TIMI 0 and 1) mortality rate gained 92-93%. Other important factors influecning outcome were also described in this study . ; Conclusion: Interventional therapy with mechanical support for left ventricle is required in cardiogenic shock. This strategy should be wide-spread used to lower mortality rate in this fatal complication of myocardial infarction.
