Title: The relationship between bone mineral density, bone metabolism markers and glycemia control in type 1 and 2 diabetes

Abstract:

The relationship between glycemia control, bone mineral density, bone quality and bone metabolism markers was estimated. The influence of BMI, age of patients, duration of diabetes on assessed variables were studied. 130 patients were enrolled into the study. 91 subjects were examined in two time intervals (after aprox. 7 months). The patients were recruited from the persons who were hospitalised due to improper glycemia control. The densitometric measurements (DXA) of lumbar spine, femur, forearm and ultrasonographic measurement of heel were performed. The level of osteocalcin, BLP, DPD and NTx were assessed. To define glycemia control the HbA1c value was measured. After improvement of metabolic control in patients with long duration of type 1 diabetes the statistically significant decrease of bone density in spine (p<0.01) and forearm was established (p<0.05). In patients with newly diagnosed diabetes type 1 the borderline statistically significant decrease in bone density in spine was observed. In women with type 2 diabetes the increase of bone density in the distal part of forearm was described. The relationship between improvement of glycemia control and bone metabolism markers was week. The strong relationship between age, BMI, and bone mineral density in type 2 diabetic patients was confirmed. Conclusions: In type 1 diabetes due to fast improvement of glycemia control th ; e decrease of bone mineral density was observed and the better BMD was connected with improper glycemia control. To avoid the decrease in bone mineral density like the worsening of the other microangiopathic diabetes complications the improvement of glycemia control should be carried out with great attention.
