TY - GEN A1 - Strzałka, Marcin N2 - Old age was considered to be the contraindication to laparoscopy in the beginning. Recently it has been suggested that laparoscopy in the elderly patients can be used safely because of the similar indications as in the younger age groups. The aim of the study was to show the possibility of the laparoscopic technique application and its advantages in the treatment of the abdominal diseases in the elderly patients. The patients over 70 years old who were treated in the 2nd Dept. of General Surgery of the Jagiellonian University between 1992 and 2001 were the subjects of the study. The patients operated laparoscopically (328) and classically (158) were analysed. The analysis included the following factors: age, sex, coexisting diseases, ASA grade, type of procedure, complication, mortality and conversion rates, volume and period of the drainage, length of hospital stay. The results of the study indicate that laparoscopy can be used widely in the treatment of the abdominal diseases in the elderly patients. Laparoscopic cholecystectomy, both elective and emergency, is the treatment of choice also in this age group. lt allows to decrease significantly the complication and mortality rates and to shorten the hospital stay. Laparoscopic hernioplasty seems to be a valuable alternative in comparison with the traditional “tension-free” methods, especially in cases or bilateral and recurren N2 - t hernias. The TAPP allows to decrease the postoperative complication rate and to shorten the hospital stay. Laparoscopy can be also used in the elderly patients to perform appendectomies, operations of perforated peptic ulcer, antireflux procedures, cardiomyotomies and liver cyst fenestrations. These laparoscopic procedures are safe and can be performed because of the similar indications as in the younger age groups.
