The study group consisted of 150 patients with rectal lesions subjected to 161 TEM operations which were performed in the 1st Department of General and GI surgery, jagiellonian University. The complications of TEM procedures occurred in 19.2% cases. There was no case of perioperative mortality due to the complications. Seventy-two TEM operations were performed for rectal adenoma. Recurrent disease was observed in 9 (12.5%) patients. The size of adenoma, its distance from the anal verge and radicality of resection had statistically significant influence on recurrence of the disease. These factor influenced in the same way recurrence free survival time. 62 patients with rectal cancer underwent TEM operations with curative effect. A total number of recurrences was 15.5%. Five-year survival in the rectal cancer patients was 92 per-cent. Only tumor size had significant effect on recurrence free time. Five-year survival rates depended on the tumor infiltration depth and tumors distance from anal verge. The comparison of patients with low-risk rectal cancer operated with TEM with the matched group of cases subjected to radical resection showed lower number of complications observed after TEM operations and shorter hospitalization time. No differences were found for 5-year survival, time free of recurre}, abstract={nce, number of local recurrences, distant metastases and cancer mortality.
