Combined application of digital dermoscopy and reflectance confocal microscopy (RCM) can improve diagnostic sensitivity and specificity in this subset of lesions. Integration of a store-andforward (SAF) telemedicine platform to exchange tele-dermoscopy and tele-confocal images can connect doctors at a distance for expert consultation and diagnosis. The aim of the first study was to improve diagnostic sensitivity particularly for the detection of featureless incipient amelanotic melanomas by comparing single reader versus double reader application. Automated referral of lesions for removal when two readers had discordant management decisions resulted in an overall sensitivity of 98.3% and specificity of 42.7%.
