The aim of this work is to determine surgical risk factors and to evaluate and optimalise surgical and perioperative procedures in group of 70 patients with ruptured DACAA operated in Department of Neurosurgery Jagiellonian University Medical College in Krakow in the period of 20 years. The group of factors which influenced the early outcome was determined after statistical analysis. There were: sex, age, clinical state on presentation, arterial hypertension, diameter of aneurysm, vascular anomalies of anterior cerebral arteries, hemocephalus, initial cerebral ischemia, temporary parent vessel occlusion during the operation. The range of subarachnoid hemorrhage significantly influenced the presence of postoperative cerebral ischemia. The subfrontal or interhemispheric route from fronto-basal craniotomy in the case of aneurysms proximal to pericallosalcallosomarginal junction is advised. The emergency parent vessel clipping is dissuaded. One stage surgery gives good results in the case of multiple aneurysms.
