The aim of the work was an attempt to answer the question if the sextant extended biopsy increases the probability of diagnosing prostate cancer when compared to sextant biopsy in patients with PSA from 4ng\/ml to 20ng\/ml. Should the choice of the kind of biopsy be related to such parameters as the volume, transverse, vertical and longitudinal diameters of the prostate\? The aim of the work was also the assessment of complications after both types of biopsy. There were 122 patients \(61 in each group\) who applied to the Clinic of Urology from 1.10.2004 and 31.03.2005. The patients who were qualified for the biopsy and had given consent for the examination were aged 50–75, their PSA level was between 4ng\/ml and 20ng\/ml, had correct results of blood–clotting test and urinalysis and had no contraindications to biopsy. After performing 61 sextant biopsies, prostate cancer was diagnosed in 14 patients. Making 61 sextant extended biopsies allowed to diagnose 17 cases of cancer. Using this type of biopsy enabled to diagnose 4 carcinomas, in which positive segment came only from the prostate periphery and would not be diagnosed in sextant biopsy. Analyzing the dimensions of prostates in extended biopsy, with the division into cancers localized peripherally and others, it was found that peripheral carcinomas were localized in prostates with bigger volume. The use of sextant extended biopsy in patients with PSA from 4ng\/ml to 20ng\/ml increased detection of prostate cancer by 21%. In the presented material significantly more frequent cases of prostate cancer in patients with PSA level in blood serum \(over 9.8ng\/ml\) were found. Patients qualified for biopsy, whose prostate was bigger than 54mm in transverse diameter or whose prostate volume was over 53.5cm³ and were qualified for biopsy, should be subjected to sextant extended biopsy. The use of sextant extended biopsy did not significantly increase the number of complications connected with prostate biopsy."]
