Search for: [Abstract = "INTRODUCTION AND OBJECTIVES Cysteinyl leukotrienes \(Cys\-LTs\) and thromboxane A2 \(TXA2\), arachidonic acid derivatives, are potent pro\-inflammatory and pro\-thrombotic mediators involved in progression of atherosclerosis and thrombus formation on unstable atherosclerotic plaque. The aim of the study was to evaluate the biosynthesis of LTs and TXA2 in MI and to estimate their prognostic value in cardiovascular risk stratification. METHODS We analyzed level of biosynthesis of LTE4, TXA2 in 183 patients with AMI \(50,82% STEMI\), as measured by HPLC with urine samples on admission, after one month and one year and their relationship with the occurrence of the composite endpoint.RESULTS We demonstrated increased production of TXA2 and LTE4 during MI. Their level decreases significantly 1 month and 1 year after MI. TXA2 was associated with occurrence of composite endpoints in short and long\-term observation. TXA2 was significantly elevated in inflammation and was associated with an increased cardiovascular risk and greater LV damage.CONCLUSIONS TXA2 has important prognostic value as predictor of cardiovascular risk in short and long\-term observation. It can be considered as indicator of atherothrombosis in CAD. Elevated levels of TXA2 in AMI and inflammation indicates production of TXA2 from extra\-platelet sources and need for pharmacotherapy that inhibits generation of TXA2 from extra\-platelet sources.
