METHODOLOGY ESC\-HF Long Term Registry was a prospective, multi\-centre, observational study conducted at 211 cardiology centres in 21 European and Mediterranean ESC member countries. The patients were recruited at the Sites on one regular day of the week for the subsequent 12 months, with patients being monitored for 12 months. 1202 patients were enrolled in the Polish part of ESC\-HF Long Term Registry during the 1st phase of the registry between May 2011 and April 2013.
