To evaluate hemodynamic effects of nitricoxide \(NO\) and silde in pulmonary artery \(PA\) reactivity test in ptswith PH due to HFREF.Methods\: 20 pts with PH due to HFREF who were previouslydisqualified from HTX because of fixed PH were enrolled to the I partof the study. Right heart catheterization \(RHC\) was performed withhemodynamic measurements at baseline, after inhalation of NO andthen after silde p.o. In the II part of research 17 pts received silde for 3months. At baseline and after 3 months of treatment with silde, clinical assessment, echo, cardio\-pulmonary exercise test, 6\-minutewalk test \(6MWT\) and RHC were performed.Results\: In pulmonary artery reactivity test there was significantreduction of PAP, PVR, TPG, SVR after silde administrationcomparing to baseline and NO. Besides, after silde increase of CI andSatPA were observed. 13 of 17 pts completed the 3\-months therapywith silde. Comparing to baseline after silde therapy there wassignificant reduction of PVR and TPG and increase of CI with nochange of PCWP and SBP. There was also improvement of peakVO2,OUES, distance in 6MWT, LVEF, PT SŚ, PT EŚ.Conclusion\: Sildenafil when compared to NO is superior to reducePVR, PAP and TPG in PA reactivity test in candidates for HTX withPH due to HFREF. 3\-month therapy with silde improveshemodynamic parameters, right and left ventricle performance as wellas exercise capacity in pts with PH due to HFREF."]
